How to Keep a Bladder Diary for Three Days
How to keep a bladder diary: for at least 3 days, write down what you drink, each trip to the toilet, any leak and what you were doing. NICE asks for a diary of at least 3 days, and Mayo Clinic starts treatment with one. It costs nothing, and it shows a clinician what a supplement label cannot.

Why
Why keep a bladder diary?
A bladder diary turns a vague worry into numbers. Memory is poor at counting trips, and a written record is not.
NICE lists a diary of at least 3 days as part of the first assessment for leaks (NICE NG123, incontinence in women). The AUA guideline for overactive bladder also uses a voiding diary (AUA/SUFU 2024, OAB guideline). Mayo Clinic starts its treatment advice with one (Mayo Clinic, incontinence treatment).
A diary also gives you a before picture. If you try training, a capsule or both, you can compare the same three days later and see whether anything changed. Without it, you only have a feeling.
The page
What do you write in a bladder diary?
You write the time, the amount and the event for each line. Keep it plain and fast, since a diary that takes ten minutes a line will be abandoned.
| Time | Drink | Toilet trip | Leak | Note |
|---|---|---|---|---|
| 7:15 | Coffee, 1 cup | Morning | ||
| 8:00 | Yes, normal | |||
| 10:40 | Water, 1 glass | Small, cough | On the stairs | |
| 11:30 | Yes, sudden urge | Almost late |
Mark leaks as small, medium or large, and note the trigger. A sneeze points to stress leaks, while a sudden need that arrives before you reach the toilet points to urge leaks (Mayo Clinic, urinary incontinence causes).
Write down night trips, too. Waking more than twice a night counts as a sign in overactive bladder (Mayo Clinic, overactive bladder).
Timing
How long should you keep a bladder diary?
Keep it for at least 3 days, as NICE advises (NICE NG123, incontinence in women). Choose days that look like a normal week, not a holiday or a stomach bug.
Three days can be in a row or spread across a week. Include one day off work if you can, since routines differ and leaks often follow routines.
Some people keep one longer and find new patterns. A week gives you a stronger average, though three days is the minimum for a clinician to use it.
Fill it in as you go, not at night. A diary written from memory at bedtime misses the small trips and the near misses, which are often the useful lines. Carry a card in a pocket or keep the notes app open on your phone.
Stick to the plain pen and paper if you like. A phone note works too, as long as you write the time of each entry as it happens.

How do you read a bladder diary?
You read it by counting and by looking for triggers. Count the trips in 24 hours; Mayo defines overactive bladder as 8 or more (Mayo Clinic, overactive bladder). Count the leaks, then match each to what you were doing.
- Blood in your urine, or passing clotsPink, red or cola-colored urine needs a check every time; clots need a provider right away.Same day
- Fever, chills, nausea or pain in your side or backWith urinary symptoms these can mean a kidney infection. Contact a provider the same day.Same day
- You cannot pass urineBeing unable to urinate needs a provider right away.Same day
- Burning, frequent small trips, urgency with an empty bladderCommon bladder-infection signs. Infections need diagnosis; cranberry does not treat them.Book a visit
- Sudden urges or leaks during normal activitiesNIDDK advises seeing a professional right away; leaks are common and often treatable.Book a visit
- Any vaginal bleeding after menopauseA reason to call a provider, whatever the bladder is doing.Book a visit
Severe?Severe pain, high fever with shaking chills or vomiting, or no urine at all: get urgent care now, or call 911 in an emergency.
Signs from MedlinePlus, CDC, NIDDK and Cleveland Clinic (MedlinePlus, Blood in urine) (CDC, About UTIs) (NIDDK, bladder control problems in women) (Cleveland Clinic, postmenopause). A supplement never replaces a check for these.
Look for gaps too. If you pass urine every hour while drinking little, that is a pattern worth showing. If most leaks follow coffee, a trial of less caffeine is advised by NICE for overactive bladder (NICE NG123, incontinence in women).
Add up each column at the end of the day. Total trips, total leaks, largest gap between trips and number of night wake-ups give you four numbers, and four numbers are easy to compare after a change.
A diary does not diagnose. It points the conversation, and the clinician decides what the pattern means.

What should you do with your bladder diary?
Take it to a clinician, and keep a copy to use for training. Frequent urges or leaks are a reason to book a visit (NIDDK, bladder control problems in women).
The diary also guides the first steps you can take at home: pelvic floor sets, bladder training and less caffeine. Our pelvic floor training guide and who it is for page explain the rest.
If you still want a capsule beside the plan, FemiCore is one a day, and the sales page shows $79 a bottle on two, $69 on three and $49 on six. Keep taking prescribed medicine.

FAQ
Bladder diary questions, answered
Do I need to measure my urine in a bladder diary?
Measuring helps but is not required for a first diary. Many people note small, medium or large and add a measuring jug if a clinician asks.
Should I change my drinking while keeping the diary?
No. Drink as you normally would so the record shows your real pattern. Changes come later, after you have read the three days.
Is there a printable bladder diary?
Yes, many clinics hand them out. A ruled page with columns for time, drink, trip and leak does the same job.
What if I forget an entry?
Write it as soon as you remember and mark it as an estimate. A rough entry is better than a gap.
