Cranberry for UTI Prevention: What the Evidence Really Shows
Cranberry for UTI prevention has moderate evidence in some groups. A 2023 Cochrane review of 50 trials and 8,857 people found about 30% fewer symptomatic, culture-confirmed UTIs with cranberry products, and fewer repeats in women with recurrent UTIs. It showed little or no benefit in care-home residents, pregnancy or poor bladder emptying, and cranberry never treats an infection already under way.

The evidence
Does cranberry work for UTI prevention?
Yes, modestly, for some people. Cranberry products lowered the chance of a symptomatic, lab-confirmed UTI in a 2023 Cochrane review of 50 trials (Cochrane 2023, cranberries and UTI).
The headline number is a risk ratio of 0.70. Picture ten infections in the groups without cranberry: the cranberry groups had about seven. In women with recurrent UTIs, the ratio was 0.74, from 8 studies and 1,555 women.
NCCIH reads the field more cautiously. It puts the drop in repeat UTIs for women at about 25%, but calls the findings inconsistent (NCCIH, Cranberry).
| Group | Result | In plain words |
|---|---|---|
| All trials pooled | RR 0.70 (0.58 to 0.84), moderate certainty | About 3 in 10 infections fewer |
| Women with recurrent UTIs | RR 0.74 (0.55 to 0.99) | About a quarter fewer; the range nearly touches no effect |
| Older adults in care homes | Little or no benefit | Not a reason to start |
| Pregnant women | Little or no benefit | Ask your obstetric team instead |
| Incomplete bladder emptying | Little or no benefit | Needs a clinician's plan |
Who gains little
Who does cranberry not help with UTIs?
Cranberry does little for older people in care homes, pregnant women and people whose bladder does not empty well. Those three groups showed little or no benefit in the Cochrane pooling (Cochrane 2023, cranberries and UTI).
The UK's NICE guideline on recurrent UTI goes further. It calls the benefit uncertain overall and finds no evidence of benefit for older women (NICE NG112, recurrent UTI).
So age and setting matter. A 30-year-old with three infections last year and a 78-year-old after menopause are not the same case. See menopause and the bladder for why risk climbs later in life.
Active infection
Can cranberry treat a UTI you already have?
No. NCCIH does not recommend cranberry as a treatment for an existing UTI (NCCIH, Cranberry). The trials looked at fewer new infections, not at clearing one.
A bladder infection usually shows as burning, frequent small trips, urgency and lower belly pressure. Fever, chills or side pain can mean it has reached the kidneys (CDC, About UTIs). Left alone, a kidney infection can cause lasting damage (Mayo Clinic, UTI).
Antibiotics, when a clinician confirms they are needed, are the treatment. Using them only when needed matters too, since resistant germs cause millions of US infections each year (CDC, Antimicrobial Resistance).

Definitions
What counts as a recurrent UTI?
A recurrent UTI means two or more infections in six months, or three or more in twelve months, in NICE's definition (NICE NG112, recurrent UTI). That is the group most cranberry trials cared about.
It is common. About half of women get a bladder infection at some point, and about a quarter of them get repeats (NIDDK, UTI in adults facts).
- NICE puts habits and hygiene first (NICE NG112, recurrent UTI)
- Vaginal estrogen may be considered after menopause if those fail
- Evidence for lactobacillus probiotics is inconclusive
- Preventive antibiotics are a later step, chosen by culture results
Mayo Clinic adds plenty of water, and says to try cranberry only if your healthcare team agrees (Mayo Clinic, UTI).
PAC
What is cranberry PAC, and is 36 mg the dose to look for?
PAC stands for proanthocyanidins, plant compounds in cranberry that many products are standardized to. FemiCore's label lists "std. to 30% proanthocyanidin".
A 36 mg PAC figure circulates widely online. None of the sources we cite sets it as a dose, so we do not use it as a target.
The arithmetic for FemiCore: cranberry is line 2 of 4 in a 350 mg blend, so at most 175 mg. Thirty percent of 175 mg is about 52 mg of PAC, at most. The test method is not named, and PAC results from different methods do not compare. That ceiling cannot be matched to any trial.

See packsJuice or pills
Is cranberry juice or a cranberry pill better for UTIs?
Neither form has been shown to win. The Cochrane review pooled many cranberry products and doses, juices and tablets among them (Cochrane 2023, cranberries and UTI).
The practical differences are everyday ones. A daily glass of juice adds sugar and volume; a capsule adds neither but hides how much cranberry you get. Large amounts in any form can upset the stomach (NCCIH, Cranberry).
Whichever you choose, keep it steady and log your infections by date. A calendar with three marks a year tells your clinician more than a feeling.
Safety
Is cranberry safe with warfarin and other medicines?
Cranberry is generally safe by mouth, but the warfarin question is open. Studies conflict, so NCCIH asks warfarin users to talk with their provider first (NCCIH, Cranberry).
Cranberry in food amounts appears safe in pregnancy; larger amounts are not well studied (NCCIH, Cranberry). Our sources do not link cranberry to kidney stones. If you have had stones, the NIDDK diet advice centers on oxalate foods and does not mention cranberry (NIDDK, diet for kidney stones (oxalate)), so ask your clinician.
Keep taking prescribed medicine, antibiotics included, exactly as directed. Cranberry is an add-on to discuss, never a swap. More on supplements and medicines.
The capsule
What does the cranberry in FemiCore add up to?
FemiCore's cranberry is a standardized extract, second of four herbs, capped at 175 mg by label order. It is the best-studied line in the capsule.
Still, the research above is about cranberry products for repeat infections, not about FemiCore and not about leaks. No trial has tested the capsule. The label's "Healthy Urinary System" is a structure/function claim, not a UTI claim (21 CFR 101.93, structure/function claims).

FAQ
Cranberry and UTI questions
Is 36 mg of PAC the right cranberry dose for UTIs?
No cited source sets 36 mg of PAC as a proven dose. PAC numbers also depend on the lab test used, and results from different methods do not line up. FemiCore's cranberry is at most about 52 mg of PAC by an unnamed method, so it cannot be matched to that figure.
Is D-mannose better than cranberry for UTIs?
Our cited sources do not cover D-mannose, so we cannot rank the two. FemiCore contains no D-mannose. For cranberry, the best summary is the 2023 Cochrane review (Cochrane 2023, cranberries and UTI). Ask your clinician or pharmacist before trying D-mannose.
Are cranberry pills or juice more effective?
No form has been shown to be better. Trials used juices, tablets and capsules at many doses (Cochrane 2023, cranberries and UTI). Juice brings sugar and extra fluid; capsules bring less certainty about the amount. Choose the form you can take steadily and discuss it with your clinician.
Should I take cranberry instead of antibiotics for a UTI?
No. Cranberry is not a treatment for an infection that has started (NCCIH, Cranberry). Burning, urgency or fever need a clinician, who may test your urine and prescribe antibiotics. Finish the course as directed, even if you also take cranberry later.
Does cranberry thin the blood like warfarin?
Cranberry is not a blood thinner, but evidence on whether it changes how warfarin works is conflicting (NCCIH, Cranberry). If you take warfarin, tell your prescriber before adding any cranberry product, capsule or juice, so your blood tests can be watched.
How many UTIs a year count as recurrent?
Three or more in twelve months, or two or more within six months, by the NICE definition (NICE NG112, recurrent UTI). If you meet it, ask for a urine culture with each episode. The results help your clinician pick a prevention plan.
Does cranberry help with bladder leaks?
No study links cranberry to fewer bladder leaks. Its research is about repeat infections. For leaks, supervised pelvic floor muscle training has the strongest evidence (Cochrane, PFMT for incontinence).
